Q: Why does my RAG system return wrong answers even with a good LLM?

Because in most cases the fault is not the model but retrieval — the LLM is answering faithfully from a chunk that retrieval should never have ranked first, and the "hallucination" ticket is really a retrieval-precision failure in disguise. The dominant cause is single-vector compression. Standard RAG embeds each chunk into one dense vector and matches it against one query vector, which is fast and cheap but lossy: a single vector is an average over the whole chunk, so it cannot preserve which specific words or facts mattered. This works acceptably for broad topical queries ("what is this document about") and fails for queries whose answer hinges on a specific token — "which clause covers early termination," "what value is in the third column," "what dosage does the protocol specify" — because the detail the query needs got averaged away, so the passage that actually contains the answer embeds to a vector that does not rank highly, and a less-relevant chunk wins instead. The model then does exactly what you asked: it answers from the context it was given, which happens to be the wrong context. A second common cause on document-heavy corpora is the parse step: PDF text extractors mangle tables, charts, and multi-column layout, so the retriever never even sees the information the user is asking about. The diagnostic discipline that fixes this is to evaluate retrieval separately from generation — measure whether the right passage is in the retrieved set before blaming the model — because teams that only look at final answers cannot tell a retrieval miss from a genuine model error, and they are usually different problems with different fixes. Once you measure, the remedy follows: better retrieval (hybrid, reranking, late interaction, or visual) rather than a better prompt or a bigger model.

Q: What is late-interaction retrieval and how is it different from normal vector search?

Late-interaction retrieval, introduced by ColBERT, keeps a separate vector for every token in a chunk instead of compressing the whole chunk into one vector, and scores relevance by fine-grained token-level matching rather than a single vector comparison. The difference is where the "interaction" between query and document happens. In standard (single-vector) dense retrieval, each document and each query is reduced to one vector up front, and relevance is a single similarity between those two vectors — all the token detail is collapsed before the comparison, which is why it is fast (you compare one vector to one vector) but lossy. In late interaction, the interaction is deferred to query time: the document is stored as many token vectors, the query is also token vectors, and the score is computed by taking, for each query token, its best-matching document token (the maximum similarity), then summing those best matches — an operation called MaxSim. This means relevance is built from precise token alignments, so a query like "early termination" can strongly match the exact region of a passage where those concepts appear even if the passage is broadly about something else, recovering the specific-clause and specific-value queries that single-vector search drops. The tradeoff is cost: storing a vector per token instead of per chunk multiplies index size substantially, and MaxSim scoring needs a retrieval engine that supports multi-vector (late-interaction) indexing and search. That is why late interaction is a deliberate precision upgrade for corpora where answer accuracy depends on specific tokens, not a universal default — you adopt it when evaluation shows single-vector search missing specific-answer queries, and you accept the storage and compute cost because the precision pays for it on that corpus.

Q: When should I use late interaction or visual retrieval versus standard RAG?

Follow the failure mode, not the fashion: start with the cheap, well-understood baseline and escalate only where evaluation proves it insufficient, because each step up in retrieval sophistication buys precision at real cost. The baseline that most RAG systems should start with is single-vector dense retrieval combined with BM25 lexical search (hybrid fusion) and a cross-encoder reranker over the top candidates. This is cheap, mature, and good enough for a large fraction of workloads — plain-prose knowledge bases, FAQs, documentation, most text corpora — and reaching past it prematurely is over-engineering. You escalate to late interaction (ColBERT-style) when retrieval evaluation on your own corpus shows specific-token queries failing on text — clause-level, value-level, or entity-level questions where the single vector is averaging away the detail that determines relevance — and you are willing to pay the increased index size and multi-vector infrastructure for the precision gain. You escalate to visual retrieval (ColPali-style) when your documents are visually rich — tables, charts, figures, complex layout — and you can demonstrate that the text-extraction step is losing the answer before the retriever sees it; there, embedding the page image is not a luxury but the only way to retrieve the content at all. The two anti-patterns bracket the decision: using ColPali on a corpus of plain prose FAQs burns storage and compute for no gain (over-engineering), while running text-only single-vector RAG over scanned financial statements guarantees wrong answers because the parser destroys the data (under-engineering). The discipline that makes the choice correct is retrieval evaluation on your actual corpus — the right technique is entirely corpus-dependent, so you measure precision per document type and let the measured failure mode select the technique, rather than defaulting to either the cheapest or the trendiest.

Q: Does late-interaction retrieval replace reranking?

No — they operate at different stages and are complementary, and a strong retrieval pipeline often uses both. The distinction is first-stage retrieval versus reranking. First-stage retrieval (whether single-vector dense, BM25, or late interaction) must score a large candidate set — potentially millions of chunks — efficiently, to pull a manageable shortlist. Reranking takes that shortlist (typically tens to low hundreds of candidates) and rescores it with a much more expensive, more accurate model. A cross-encoder reranker feeds the query and each candidate document jointly through a transformer to produce a precise relevance score, which is far more accurate than any first-stage method but far too slow to run over the whole corpus — so it is a reranker, not a retriever. Late interaction sits in the first stage: it is more precise than single-vector dense retrieval because it matches at the token level, but it is still a first-stage method designed to score many candidates efficiently (MaxSim is cheaper than a full cross-encoder joint pass). So they compose: late interaction can be your first-stage retriever that produces a higher-quality candidate set than single-vector search, and a cross-encoder reranker can then reorder that set for final precision. In practice, adding late interaction improves what the reranker has to work with (better candidates in, better results out), and it can reduce reliance on the reranker to rescue misses that single-vector retrieval would have made — but it does not eliminate the value of a final reranking pass on precision-critical applications. The right combination depends on your latency budget and precision requirements: some systems run hybrid dense+BM25 → rerank; others run late interaction → rerank; the most demanding run multiple first-stage retrievers fused, then rerank.
