Q: What is a SIM swap attack and why does it bypass multi-factor authentication?

A SIM swap is a social-engineering attack on the mobile carrier, not on your application. The attacker gathers enough personal data on the victim to pass the carrier's identity check, then persuades a store, call-centre agent, insider or dealer portal to move the victim's phone number onto a SIM the attacker controls, or ports the number to another carrier entirely. From that moment every SMS one-time password, voice callback and password-reset link sent to that number arrives with the attacker. Multi-factor authentication that uses the phone number as its second factor is therefore defeated without touching your infrastructure: your OTP service delivered correctly, your logs show a clean success, and the victim's only clue is that their handset has lost signal. The compromise is at the carrier, which is why the defence has to stop trusting the number rather than harden the OTP.

Q: Does NIST still allow SMS one-time passwords as an authenticator in 2026?

Yes, but as a restricted authenticator. NIST SP 800-63B-4, finalised in July 2025, keeps out-of-band authentication over the public switched telephone network — both SMS and voice — in the restricted category, which means agencies and organisations that use it must offer alternatives, assess the risk, and inform users. Revision 4 also adds explicit guidance that verifiers should consider risk indicators such as device swap, SIM change, number porting and other abnormal behaviour before using the PSTN channel. In practice that turns the phone number from a standalone factor into a channel you only use after checking with the carrier that the number has not just changed hands, and only for actions whose value justifies the residual risk. It is not a ban on SMS; it is a demotion of SMS to a supplementary, risk-gated role.

Q: How can an application detect that a phone number has recently been SIM-swapped or ported out?

Carriers know the exact moment a SIM or port event happened, and that data is increasingly exposed through standard APIs. The GSMA-backed CAMARA project defines a SIM Swap API that returns whether a number's SIM changed within a requested window and, where the operator supports it, the timestamp of the last change. Aggregators such as Twilio expose the same data through their lookup products, subject to per-carrier registration and country coverage. In India, the Department of Telecommunications' Digital Intelligence Platform publishes a Financial Fraud Risk Indicator that classifies a number as medium, high or very-high risk using inputs from the cybercrime reporting portal, the Chakshu platform and bank reports, and the 2025 amendment to the Telecom Cyber Security Rules introduces a Mobile Number Validation platform. The architectural rule is to call this signal at the moment of the sensitive action — a payee addition, a recovery, a number change — not once at enrolment, and to budget it on the hot path with short caching so the answer is recent.

Q: What should happen when the SIM-swap check returns a hit — should we block the user?

Not a hard block. A recent SIM change is a strong signal but also the normal experience of anyone who lost or replaced a phone, so a dead end produces support tickets and churn while teaching the fraud team to disable the check. The pattern that works is a cooling-off period: for a defined window after a SIM or port event, typically 24 hours, high-value actions such as adding a beneficiary, changing a withdrawal address or completing an account recovery are held, the user is offered a stronger or assisted verification path, and the previous device, the registered email and any secondary contact are notified that the number changed and that an action was attempted. Legitimate users rarely need to add a new payee in the first hour after replacing a phone; attackers almost always do. Turning their one-hour window into a delayed, multi-channel-visible process defeats the attack without punishing the honest case.

Q: Which authentication factors actually survive a SIM swap?

Anything bound to something the attacker did not obtain by moving the number. Passkeys and other FIDO2 credentials survive because the private key is scoped to your origin and lives on the user's device or in their platform account, so the phone number is irrelevant. Authenticator-app TOTP survives because the seed is on the enrolled device, though it remains phishable and does nothing for recovery. Push notifications with number matching survive because they go to an enrolled app with a bound key, not to a number. Device-bound signing keys generated at enrolment survive and are the right basis for transaction approval in banking and payments. Email magic links survive only if the mailbox itself cannot be recovered by SMS, which is frequently not the case. SMS OTP and voice callback do not survive at all, and voice adds call-forwarding as a further weakness. The design question is which of these factors each flow — login, step-up, recovery — is permitted to rely on.

Q: What do the RBI's 2025 authentication directions change for Indian payment providers?

The Reserve Bank of India (Authentication Mechanisms for Digital Payment Transactions) Directions, 2025, issued on 25 September 2025 and effective from 1 April 2026, replace the historical reliance on SMS OTP with a technology-neutral framework. Every digital payment must use at least two distinct factors, and for non-card-present transactions at least one factor must be dynamically created or proven for that specific transaction. SMS OTP is explicitly not mandated; issuers may use any factor that meets the principles, and they may add behavioural and contextual checks such as device attributes, location and transaction history. Crucially, if a loss arises from a transaction effected without complying with the directions, the issuer must compensate the customer in full without demur. For architects that shifts the calculation: an SMS-only approval flow that a SIM swap defeats is now both a security weakness and an uncapped liability, and the directions give explicit room to replace it with device-bound and risk-based approval.

Q: Why is account recovery the real target of SIM swap rather than login?

Because recovery is the flow designed to work when the user has lost everything else, which is precisely the situation an attacker wants to simulate. A first-factor login with a passkey is hard to defeat, but a password-reset link sent by SMS, a "we will call you to verify" support flow or a recovery-code delivery over the phone number bypasses the passkey entirely. Attackers therefore rarely bother with the front door; they reset the password, mint a fresh session, change the registered email and phone number, and then move funds. The defence is to treat recovery and number-change as high-assurance actions carrying the same requirements as the actions they unlock: a second enrolled authenticator, a verified-identity flow with liveness and document checks, or a deliberately slow assisted path with delay and multi-channel notification. Slowness is a feature in recovery because the attacker's advantage is speed.

Q: What do the FCC's SIM swap and port-out rules require, and do they solve the problem for service providers?

The FCC's Report and Order in WC Docket 21-341, adopted on 15 November 2023, sets baseline obligations on US wireless providers: they must use secure methods of authentication before redirecting a customer's number to a new device or provider, immediately notify customers whenever a SIM change or port-out request is made on their account, offer account locks, maintain procedures for failed authentication attempts, and train employees to identify and prevent SIM swap and port-out fraud. These rules raise the cost of the attack, and comparable measures exist in other jurisdictions, but they do not solve the problem for a bank, exchange or SaaS platform that relies on the number. Carriers still make mistakes, insiders are still bribed, and rules apply per country while your users roam. The service provider's security boundary has to be designed on the assumption that the carrier will occasionally hand the number to the wrong person, and the carrier notification is best treated as one more signal feeding your own risk decision.
