Q: What is audio prompt injection against a voice agent?

Audio prompt injection is any sound in the call that the agent turns into an instruction it was not supposed to follow. It can be a plainly spoken command from a second speaker, an instruction hidden under the caller's own speech so that it piggybacks on legitimate audio, or an imperceptible adversarial perturbation that an audio-language model decodes as text while a human hears noise. Research published in 2026 (AudioHijack in April, Piggybacking on Perception in July) showed both the imperceptible and the concurrent variants working against current models and, in AudioHijack's case, against commercial voice agents from Mistral AI and Microsoft Azure. The defining property is that the attack reaches the model through the same channel as the user, so no input filter that runs on text can see it before the model does.

Q: Can a voice agent authenticate a caller by recognising their voice?

Not as an authentication factor. For customers without an app, a callback to the number on file gives moderate assurance for lower-value actions, provided the number was not supplied during the call and carrier signals show no recent SIM change or port-out. SMS one-time codes are weaker still because a SIM swap captures them and a cloned caller will simply read the code back. Knowledge questions identify a caller; they do not authenticate one.

The model proposes a tool call; a policy engine decides whether the call is allowed given two inputs the transcript cannot alter: the risk tier of the action and the assurance level of the session. No. Liveness challenges were designed against replayed recordings, and a live speech-to-speech model reads random digits as fluently as a human does, in the cloned voice, with no delay a caller would notice. The same applies to spoken passphrases and to reading back a one-time code: any verification whose proof is something the caller says is something the attacker's model can say. Liveness scores and anti-spoofing classifiers can still contribute to a risk score, but they are inputs to the decision about when to demand step-up, never a substitute for a factor that lives outside the audio channel.

Q: Why should approval from a step-up be bound to a single action rather than the whole session?

Because a session-wide grant turns one legitimate approval into authority for every tool call that follows it, including calls that an injected instruction produces three minutes later. If the customer approves a push that says only "Approve this call?" at the start, the policy engine then sees a high-assurance session and allows a transfer the customer never asked for. Binding the approval to the action and its parameters means the push shows "Transfer 2,000 to account ending 4471", the approval carries those parameters, and a substituted destination or amount fails the match. It also produces a clean audit record: each consequential action has exactly one approval that names it.

Q: What is the difference between DolphinAttack-style inaudible commands and 2026 audio injection research?

DolphinAttack (2017) modulated voice commands onto ultrasonic carriers that a phone's microphone hardware demodulated into audible-range signals, so the attack targeted the sensor and needed the attacker to be physically near the device. The 2026 work, including AudioHijack and Piggybacking on Perception, targets the model rather than the hardware: it optimises perturbations or concealed instructions that audio-language models interpret as commands while remaining imperceptible or innocuous to a human listener, and it works through an ordinary narrowband phone line. The consequence for defenders is that microphone-level filtering does not help, and the defence has to be architectural: what the model is allowed to do, not what it is allowed to hear.

Q: When should a company not deploy a voice agent that can take consequential actions?

When its customer population cannot complete an out-of-band step-up. If most callers have no enrolled app and no passkey, the only step-up available is a callback to the number on file, and callbacks fail in exactly the case that matters most: an attacker who has taken over the number through a SIM swap or port-out. In that situation the defensible scope for the agent is read-only and low-risk actions, with a human handoff for anything that changes an address, resets a credential or moves money. That is a smaller product than the demo suggested, and it is the one that survives contact with a fraud ring. Expanding scope is a function of app enrolment, not of a better clone detector.
