Left alone, a concurrency spike of 1,000 functions is 1,000 connection attempts — the exact shape max_connections exists to refuse. The pattern that works has three parts. First, per-function connection count of exactly 1: a pool inside a Lambda is a trap, because the pool multiplies by concurrency (a pool of 10 at 500 concurrent executions is 5,000 demanded connections); initialize a single connection outside the handler so warm invocations reuse it. Second, a transaction-mode pooler between the fleet and the database — RDS Proxy on AWS (managed, IAM-auth-capable, failover-aware, priced around $11 per vCPU per month), or the platform pooler if you are on Supabase (Supavisor) or Neon, where pooled versus direct is just a different connection string. Q: Do I need PgBouncer if my ORM already has a connection pool?

Almost certainly yes, once you run more than a couple of instances — because the ORM pool and the external pooler solve different halves of the problem. The external pooler (PgBouncer, RDS Proxy, platform poolers) polices the aggregate: the whole fleet's client connections multiplex onto a small server-side working set, and fleet growth changes queueing at the pooler rather than pressure on the database. The two compose naturally — modest per-instance pools for request-level reuse, one transaction-mode pooler in front of the database for fleet-level multiplexing. One configuration note when stacking: keep the ORM pool small (5-10) and let the pooler do the heavy multiplexing, and make sure ORM-level prepared-statement settings match the pooler's mode. The single-instance monolith with stable concurrency is the one honest case where the ORM pool alone suffices.

Q: When is connection pooling not enough?

When the constraint is work, not connections. A pooler multiplexes access to the database's capacity; it does not add any. The tell-tale signals that you have outgrown what pooling can fix: pool checkout wait times climbing while the database's active connections sit at the sized maximum and CPU or I/O is saturated — the pool is correctly full of genuinely busy connections; queries that are slow even in isolation (that is an indexing and query-plan problem wearing a connection costume — fix the missing index before buying hardware); a working set that no longer fits memory, showing up as read latency the pool cannot influence; or write volume beyond what one primary sustains. Each has its own escalation path, and pooling buys you the calm to take it deliberately: slow queries go to query and index tuning first, since the majority of "we need a bigger database" incidents die there; read-heavy workloads go to replicas with their own pools, and heavy analytics gets routed away from OLTP entirely; big tables with retention pain go to partitioning; and only genuine write-volume or working-set exhaustion justifies sharding, which is an architecture project rather than a configuration change. The diagnostic discipline: with a pooler in place and sized, the pool metrics themselves become your capacity gauge — sustained pool-full with busy connections is a true capacity signal, while pool-full with idle-in-transaction sessions is still an application bug. Without the pooler, both look identical: connection errors.
